Medical Emergencies: If a client is experiencing a medical emergency, they should call 911, not contact the center.
Medication Refill Policy: The client will need to be examined before any refills are provided, including controlled substances.
Client conduct & Safety: Alora Health reserves the right to terminate a session or ban a client for inappropriate, disrespectful, or harassing behavior. Clients to disclose all allergies, medications, and physical conditions before treatment to ensure safety. The Center and its providers offer guidance and recommendations; however, the client is advised and encouraged to clarify any questions regarding their treatment plan with their primary care physician.
Cancellation Policy: Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the providers’ day that could have been filled by another client.
We require 24 hours notice for any cancellations or changes to your appointment. Clients who provide less than 24 hours notice will be charged a $50 fee.
Failure to reschedule or show up for the appointment results in the full appointment fee being charged to the credit card on file.
Financial Policy: Alora Center for Health & Wellness operates on a cash-pay basis, the client is directly responsible for all fees incurred for services rendered. The client agrees to pay for services at the time of the visit, unless other arrangements have been made in writing.
Alora Center for Health & Wellness does not bill insurance companies directly, and it is the clients responsibility to seek reimbursement from their insurance provider, if applicable.
Fees may be subject to change, and the client will be informed of any significant changes prior to receiving services. The client is responsible for any additional charges for medications, supplements, procedures, tests, supplies, or other services not included in the standard consultation fee. There are no refunds for services rendered.
Independent Contractor Status & Consent to Provider Care: Alora Center for Health & Wellness (the “Center”) provides administrative services and a facility for healthcare providers to practice. The healthcare providers performing services at the Center—including Nurse Practitioners—are independent contractors and are not employees or agents of the Center. Each provider is solely responsible and liable for the care they provide to the client, and the Center is not responsible for the individual clinical judgment or medical treatment rendered by these contractors.
